Two Rivers: Community Health Assessment
One of the core functions of Public Health is Assessment. Two Rivers Public Health Department is currently working through the MAPP process (Mobilizing for Action through Partnerships and Planning).
• MAPP is a community driven, strategic planning tool, utilizing four assessments to identify priority health challenges and opportunities that the community may be facing
o Community Themes and Strengths Assessment: Provides a deep understanding of the issues residents feel are important, including how quality of life is perceived in the community.
o Local Public Health System Assessment: This involves a comprehensive assessment of all of the organizations and entities that contribute to the publics’ health. The public health system includes hospitals, schools, service organizations, law enforcement, etc.
o Community Health Status Assessment: Identifies priority issues related to community health and quality of life.
o Forces of Change Assessment: Focuses upon the identification of forces such as legislation, public policy, technology, and the social-economic trends that all impact the community and its public health system.
• From the data collected the steering committee will identify the top five strategic issues and formulate a Community Health Improvement Plan
• Two Rivers is asking that residents of Buffalo, Dawson, Franklin, Gosper, Harlan, Kearney and Phelps County complete the Community Health Survey.
o Two Rivers Public Health Department will be at each County Fair and the survey can be completed there.
o Those who complete the survey will be entered for a chance to win 1 of 7 $50 gift cards.
